A Discount Plan for Patients with No Insurance
The 38th Street Dental "Gold Plan" is a discounted fee schedule for you, the patient with no dental insurance. For a modest annual fee of $49, you will be assigned to the Gold Plan fee schedule that will give you 20% off our regular charges. This can mean a huge savings if you find yourself in a situation where you need restorative care, emergency care, or need help maintaining your routine care schedule. In addition, if you attend two routine care visits for exam, bite wing x-ray pictures, and routine adult prophylaxis, your savings will more than offset your $49 annual membership fee.

What will I pay for treatment?
Outlined below are a sample of the discounted fees you will benefit from with enrollment in the Gold Plan. Please remember that ALL procedures and their associated codes are prescribed by the doctors and that a patient's individual condition will dictate prescription of the code and procedure deemed most appropriate by Dr. Meek or Dr. Tasch.

What are the Limitations?
As with all things in life, there are restrictions with the Gold Plan. We have designed this plan to be straight forward, but there are a few limits and restrictions with our offer.
1. Patients with any form of third party reimbursement are not eligible for discount extended through the Gold Plan.
2. Patients that acquire insurance or third party reimbursement during their period of coverage under the Gold plan will not be eligible for the benefits of the plan from their active date with the plan forward.
3. Payment through healthcare savings accounts or flex plans will be allowed under the terms of the Gold plan.
4. Membership fee is not transferable and qualifies only the patient applying for the plan’s benefits. In case of families, each family member must purchase membership.
5. Membership term is for twelve months and will commence on the day of payment for benefit under the terms of the Gold Plan.
6. The membership fee is not refundable if the patient moves during the 12 month membership period.
7. The membership fee is not refundable if the patient receives dental benefits or third party payment coverage during their period of membership.
8. The Gold Plan discount does not apply to bleaching kits, tooth brushes, orthodontics, or elective cosmetic procedures such as composite bonding or veneers.
9. Terms of the Gold Plan offer may be altered or updated at any time throughout the membership period.
10. Procedures may be referred to a specialist due to clinical complexity or scope of practice concerns. Should referral occur,
discounts will not carry to specialists offices. Drs. Meek and Tasch reserve sole right to refer as they see fit for any condition or procedure.
